The Screening Community Health Services Employment Application

Community Health Services is licensed by The State of Arizona, Department of Behavioral Health Services as an approved DUI service agency under ARS § 28-692 and a behavioral health service agency, outpatient clinic. Community Health Services is licensed to provide individual, group, family counseling, domestic violence and DUI screening, education and treatment programs.

Community Health Services provides complete drug and alcohol screenings to court ordered DWI/DUI offenders, Arizona Department of Motor Vehicles clients, and any other defendant in need of Substance Use/Abuse assessment.

The goal of the alcohol screening is to assess the extent of a client's current and past alcohol abuse and/or dependence. Offenders are screened through an in-depth personal interview, using the latest multi-dimensional interview "tools." Objective assessment instruments are administered by qualified professionals. Referrals are made to appropriate education and/or treatment programs.

Community Health Services' cognitive behavioral approach to substance abuse assumes that individuals with addictive problems are able to assume responsibility for changing their behavior. During the screening, a feeling of rapport and trust is established between the client and the therapist. Therapeutic communication, motivational interviewing, and awareness of client's change state are essential to the therapeutic relationship, motivational enhancement, and preparing the client for Cognitive Behavioral Treatment.

Screenings are conducted by appointment Monday through Saturday, 8:00 a.m. - 5:00 p.m. The Screenings are approximately 1-1½ hours in length and consist of an in-depth interview by a skilled professional and the administration of an appropriate, approved assessment instrument.

Community Health Services meets the special needs of clients through referral to appropriate community resources. It is Community Health Services' policy to maintain for seven (7) years complete and confidential documentation of the screening results for all participants.
